Class Absence Form
This form must be submitted to the TAMS Academic Office within 36 hours of your absence. If the absence occurs on a Friday you must submit it by 12:00 noon the following Monday. If you are ill and unable to attend class, you should contact your parent/guardian immediately and also seek an appointment at the UNT Health Center. In order for the absence to be excused for illness, your parent/guardian must contact your academic counselor by 5pm on the date that the absence occurs. If not, the absence will be unexcused and you will receive the appropriate consequences for missing class. Faculty will also be reporting absences. If you have failed to report an absence that is reported by your instructor you will be given additional consequences for failure to attend class.
