Charitable Contribution Submission

The TAMS Administration would like to keep an eye on how much money, supplies, clothing or other items are raised from the TAMS students and given to Charity. It's nice to have those figures on hand from year to year in order to be able to brag about the work our students and clubs do. Please fill out the form below within 14 days after your contribution has been submitted to charity.

Please note the first and last names of the individuals behind the organization of the charitable event (not all the volunteers though).
When did the drive/fundraiser begin?
When did the drive/fundraiser end?
What organization was the donation submitted to?
Files must be less than 2 MB.
